Nurse - Telehealth

and participating actively within their communities.
We are seeking a Registered Nurse with the
ability to support Telehealth care services. The successful applicant will be
based in Townsville and will be part of an exciting pilot project delivering
telehealth care services to older Aboriginal and Torres Strait Islander people
aged over 50 years.
Training in the use of telehealth services
will be provided to the successful applicant. Remuneration includes significant
salary sacrifice incentives.
Professional & Personal Skills:

The registered nurse takes a leadership role in
the delivery of telehealth care services including direct care planning for
patients, set-up of telehealth care equipment ill patient's homes, support to
patients to utilize telehealth care equipment and remote monitoring of
patients' health and provision of both in fierce
·
The registered nurse provides evidenced based
and culturally sensitive nursing care to older Aboriginal and Tones Strait
Islander patients who are aged over 55.
·
The registered nurse assesses, plans, implements
and evaluates nursing care in collaboration with individual/s and the
multidisciplinary health care team so as to achieve goals and health outcomes.

The registered nurse provides care in a range of
settings and modifies practice according to the models of care delivery.
·
The registered nurse takes a leadership role ill
the coordination of nursing and health care within and across different care
contexts to facilitate optimal health outcomes. This includes appropriate
referral to, and consultation with, other relevant health professionals,
service providers, and community and support services.

The registered nurse actively participates in
and contributes to the objectives of the pilot project within a small project
team including support of evaluation activities and stakeholder engagement
activities.

The registered nurse contributes to the
organization's development of best practice telehealth care
Requirements:
·
Current up to date registration with AHPRA
·
Current drivers licence
·
Demonstrated experience working with Aboriginal
and Torres Strait Islander patients and communities
·
Clinical problem solving skills to maximize
patient care
·
Demonstrated ability to relate to aged people
